In doing our part to prevent the spread of COVID-19 in our community and workplace, we are restricting access to our facility to anyone who either may have been recently exposed to, or is demonstrating symptoms of the virus.

When you arrive for your appointment, we will be taking your temperature. We do require all clients and staff to wear a facemask upon entering, and at all times while in our facility (masks will be adjusted as necessary for facial work). Please bring your own mask, as we do not provide them. You will not be admitted without a mask.

1. You have, in the past 14 days, had any close contact with anyone who is either confirmed or suspected of being infected with COVID-19.
2. You have traveled out of the country in the last month.
3. You currently experience or display, or have experienced or displayed any of the following symptoms:

• Elevated temperature or fever of 100.4 F or higher
• Cough
• Shortness of breath or difficulty breathing
• loss of smell and/or taste
• Fatigue, muscle aches, chills, shaking
• Persistent headaches
